What is Social Prescribing?

 

Social Prescribing is a service that GPs, nurses and other healthcare professionals can refer to in order to provide patients with additional services and support to improve their health and well-being.

At the Aberfeldy Practice we provide Social Prescribing in collaboration with other local practices in the Poplar and Limehouse Health and Wellbeing Network. The service offers one-to-one appointments where people can talk about their concerns and the factors that affect their health and get support to identify the best assistance.

Social Prescribing also helps healthcare professionals care for their patients, as so often, the factors influencing health are complex in nature and cannot always be solved with a prescription for medication.

Social Prescribing services are popping up across the country and have proven so far to lead to several positive outcomes for the people who use them, including:

  • Helping people feel healthier
  • Making new friends
  • Helping people to get back to work or into training and education, including a full range of activities at our new local community centre on the Aberfeldy Estate, Feldy
  • Linking people to projects where they can make a difference to their local community, including local nature based projects at Cody Dock and R-Urban
  • Helping people navigate and find the right help with benefits, housing, legal and debt advice
  • They might also refer you to a health coach

Our Health and Wellbeing Coaches work together with our social prescribers and can provide our patients with 6 one-to-one coaching sessions to help you build the knowledge, confidence and skills to manage your health conditions and improve your wellbeing. By focusing on what’s most important to you, a health and wellbeing coach will work alongside you to build your knowledge, confidence and skills to manage your health conditions and improve your wellbeing.

 

How will a health coach help me with my health & wellbeing? 

Health and wellbeing coaches offer you one-to-one sessions and they help put what matters to you, first. They do this by:

  • Listening to what’s important to you without judgement
  • Providing a space for you to explore the thoughts, feelings and behaviours that might be holding you back
  • Helping you to create goals that you identify for yourself
  • Signposting you to find the appropriate advice, guidance and support you need
